Ph.D. Program - Health Insurance

University students must have health insurance coverage. The university offers two health insurance plans with coverage beginning on September 16. Click here to view the 2004-2005 health insurance plan descriptions and here to view more information, including costs.

Booth Ph.D. students must have health insurance coverage effective September 7 in order to participate in Orientation activities (including Math Camp).

To accommodate students' need for a health insurance option prior to September 16, the university offers a one-month Early Arrival Insurance plan with coverage beginning on August 16.

Here are some important details about enrolling in the Early Arrival Insurance option:

  • You need to enroll in this option if you are not applying for the health insurance waiver at the university
  • The plan covers only the one month period between August 16 and September 15, 2004
  • If you enroll in the Early Arrival Insurance plan, then you must also enroll in one of the university health insurance plans for the 2004-2005 academic year.

Enrollment information for the Early Arrival Insurance option is available here.
