The State of Illinois
requires all students entering a college or university in Illinois to
show proof of vaccination against certain vaccine-preventable diseases
before registration. This includes: Measles (Rubeola), Mumps, Rubella
(German Measles), and Tetnus/Diptheria. Proof of immunity varies from
disease to disease and also depends upon your citizenship status. Click

Documentation
There are two ways
to demonstrate proof of vaccination:
Have your physician
complete the Immunization Requirements
form and provide any necessary documentation.
OR
Mail or drop off
a copy of your medical history that contains a copy of your childhood
immunizations showing that you meet the Immunization
Requirements.
To facilitate processing,

Students with incomplete
records after this date will have a restriction placed on their university
account which will deny you access to university facilities and to winter
course registration
and bidding.
Immunization
Clinics
Immunization clinics
will be held at the Student Care Center (SCC) for students from Septmember
20-25, 2004 to help in meeting this requirement. Details of the immunization
clinics are available at http://scc.uchicago.edu
. There is a fee for immunizations.
